- Drivers license or Identification Card
- Current physical within the last year
- Current 2 step ppd within the last year
- Diploma, Transcript or GED
- Child Abuse Clearance (we can run if you do not have one)
- Social Security Card
- DPW Residential testing certificate (if you do not have one you can take the test at the link provided (services.dpw.state.pa.us/pch_comptest)
- Car insurance if you drive
Certified Nursing Assistants, Licensed Practical Nurses and Registered Nurses will require the following documentation:
